Applicant must be a High School Senior.

Applicant must have completed at least two years in a 4-H or FFA Livestock Project.

Applicant must submit a current high school transcript INCLUDING senior year to date.

Applicants must submit three letters of recommendation meeting the below criteria:

• 1) A high school-associated letter from a teacher, guidance counselor, or principal.
• 2) A 4-H/FFA letter from an FFA advisor, 4-H extension agent, or 4-H club leader.
• 3) A Jacoby Feed letter from an authorized Jacoby Feed dealer.

Applicant must attach a response to the essay question included in the application.

Applications must be printed or typed.

Applicant must attach a current photo to be considered.
